For In-Office Counseling

Directions: For directions to my office please visit my Contact Page, Scroll down to the map, click view larger map (link is below my name inside the map), beneath my picture you will see "Directions", click on directions and in the box next to the magnifying glass you can enter your address to get directions.

Forms: Please go to the Forms page. Please download, print out & fill out the New Client Forms & Initial Questionnaire. Please either scan or take a picture of the forms with your phone & email them to JReinach@JMRCounseling.com , Fax them to 813-886-1332 or simply bring them to your first session.

For Telephonic, Video or Chat Counseling

Insurance: If you are using insurance, please email JReinach@JMRCounseling.com or call or text me at 813-629-6890 to let me know the insurance company name and your member id.

Schedule: To schedule an appointment please visit our Scheduling Page Scroll down and click book now and follow the on screen prompts to schedule your session. Sessions are 45-50 minutes in length.

Connecting: Once you have scheduled you will receive a 9 digit secure pin. to connect click Join a Session or go to www. Securevideo.com and click "Join a Session" & enter your 9 digit pin.

Forms: Once you have joined a session please click on the "Session Forms Icon" & complete, digitally sign & securely submit your forms. You must complete ALL Forms including: Session Forms & Online Counseling Consent Form.

Software: To secure our Video or Chat session for privacy please install the ZOOM Software. You should see "Install Zoom" icon . For Step by step picture by picture instructions are located here Links to Instructions Once you install ZOOM you will not need to reinstall for future sessions.
