To make an appointment you may

1. Call - Call Jim's cell phone first at 813-629-6890. If he is unavailable, please try the office at 813-251-2922. You may leave a message at either the cell or the office number.
2. Email - Email jmrcounseling@gmail.com to set up your appointments as well.
3. Fax - Fax a request to 813-886-1332 with the subject Appointment.

When leaving a message, whether by phone, e-mail, or fax, please include your name, the time and date desired, and a way for Jim to contact you to confirm your appointment. We will respond to you in one business day.

For the first appointment please fill out the initial session forms on this website and submit them securely electronically. If you are unable to do so, please arrive 10 minutes early for your appointment to fill out the necessary forms. You can also print them by clicking here
